The following documents are MANDATORY in order to be considered for GO Project Programming.

1. The GO Student Application Form 2010-2011 (completed by parents/guardian)
2. Academic release form (completed by parents/guardian)
3. Student Evaluation Form (completed by the child’s public school teacher)
4. Copy of the student’s most recent school report card (submitted by either parent/guardian OR school)
