Applying to GO

The following documents are MANDATORY in order to be considered for GO Project Programming.
  1. The GO Student Application Form 2011-2012 (completed by parents/guardian)
  2. Student Evaluation Form (completed by the child’s public school teacher)
  3. Copy of the student’s most recent school report card (submitted by either parent/guardian OR school)

All documents must be submitted by the application deadline of Monday, March 7, 2011. Applications can be faxed to 212-533-5012 or mailed to:
